Descartes And Locke: The Matrix Computer programmer Thomas A. Anderson leads a secret life as a hacker under the pseudonym Neo and wishes to learn the answer to the question What is the Matrix? The mysterious underground hacker offers him the opportunity to learn the truth about the Matrix. Morpheus offers Neo an option between two pills: A blue pill which would get him back in his original time, and a red pill that would let him to find the truth he is looking for. Neo chooses the red pill. Morpheus tells Neo that right now is the year of 2199 not 20th century, and that people have a war with smart robots that were made by people created in the 21st century. Morpheus thinks that Neo is that the One. He thinks that Neo will be able to put the war the end with his unlimited power. Neo is educated as a tough fighter. An opening in the back of Neos skull connects him directly to the Matrix. All the necessary knowledge and information is transferred through it. Morpheus is sure that once Neo fully gets his own power, t he Agents will be out of competition for him. Then they find out that they were revealed by their colleague, who had chosen to live in the Matrix not the actual world, and had made a deal with the Agents to give them Morpheus in exchange for a permanent come back to the Matrix. Morpheus is jailed. Neo and Trinity come back to the Matrix and try to get him out of the prison. Agent Smith kills Neo. In the actual life, Trinity says softly to Neos body what the Oracle had said to her: that she would get the true feelings of love with the One. She doesnt want to believe that he is dead and kisses him. Neos heart started beating again, hes alive; the Agents fire at him, but he moves his hand up and stops their bullets in mid-air. Then Neo destroys him. At the end he promises to show the imprisoned people a true life. Philosophy is concentrated on investigation of truths. In the movie, the investigation for the truth was about our own being. In this movie thoughts of Descartes and Locke were often used. Neo symbolized Descartes. He doubts all the things. This movie stressed the use of machinery as the course of action for humans. Descartes believed that all moves were of mechanical origin, but the spirit cannot be restricted. One of the events was a combat between Neo and Morpheus in a kung fu session. Neo is thought to be a super fighter. Initially he fails because he tries to use his brainpower to control his moves. Then he understands that the mechanism is guiding him not his brainpower. He thoughts that the human is an ideal computerized thing. Throughout the movie, many references were made to the idea of senses. It is believed by every human that to know reality is to experience through your taste, smell, touch, hear, and see. Locke believed that this was true. There is nothing in the mind except what was first in the senses. Locke believed our mind is an empty slate. This idea parallels to the ideas in the Matrix about the mind being an empty room. Locke, though, insists that when we begin to use our senses we start to have ideas. How are we to know that our senses are not programmed? There is no correct answer. No one knows. Locke does state that the mind at birth is the blank slate. In the movie, Neo was reborn into reality. He had wires and cords to machines. When they released them, he had no control over anything. He had no understanding. The world was new to him. Neo eventually learned everything again. He was taught through computers, though, he never used his senses. Locke feels that the connection of the mind an d soul is through them. The brutal attacks/bombings this week in New York, and Washington D.C., along with threats of attacks there and elsewhere in the country have changed our times forever. While the mass media concentrates on the details of the destruction, and the blanketed words of politicians, I will attempt to understand and explain the events from the fence. BOMBING AND BEING BOMBED ARE THE SAME THINGS ON DIFFERENT SIDES OF THE FENCE. Terror is not a spontaneous human action without credence. People just donââ¬â¢t hijack planes and commit harikari (suicide) without any weight of thought to the action. No one in the media seems to ask WHY DID THESE PEOPLE DO THIS HORRIFIC ACT OF VIOLENCE AND DESTRUCTION? To be able to understand the answer to this, we must first look at our U.S. Mideast Policy. During most of the 20th century, U.S. businesses have worked on attaining oil rights and concessions from countries in the Middle East and Eastern Europe. After WWI, secret back door deals by our State Dept. yielded oil rights from then defeated Turkey to fields in what is now Iraq and Saudi Arabia, in return for looking the other way at a crime against humanity, the Genocide of the Armenians by the Turks. Oil profits have been the motivating factors behind many attempts at counterinsurgency of democratic regimes by the CIA and the U.S in the Middle East (such as Iran in the 1950s, where the Shah replaced the Prime Minister who refused to give up oil rights to the U.S., and since the people couldnââ¬â¢t deal with the Shah, an extremist government headed by the Ayatollah Khomeini ultimately prevailed). During the Iran-Iraq war, America supplied both sides with weapons and advice. These are n ot the actions of a rich superpower wanting peace. Letââ¬â¢s not forget that Saddam Hussein, before being Americaââ¬â¢s vision of the Anti-Christ, was a close ally of the U.S., and the CIA. So what was the firm belief system of consecutive American administrations that caused all this to occur ? PEACE IN THE MIDDLE EAST WILL LEAD TO HIGHER OIL AND GASOLINE PRICES. Letââ¬â¢s not also forget the power of the Arms industry, disguised as defense, that still sells billions of dollars of weapons to the area. Peer Review of Strategic and Implementation Plans Essay
Team members provided feedback on the individual draft papers posted. The assignment was to provide a strategic plan for our chosen organization. The plan needed to include implementation for objectives, functional tactics, action items, milestones and deadlines, tasks and task ownership, and resource allocations. Organizational and management changes, financial forecasts, and risks with contingency plans also needed to be addressed. One of the draft papers contained excellent breakdowns on the financials for a startup company along with specifics on which participating individuals would be responsible for key action items. The budget and sales forecast were realistic for what would be expected with a new business. The identified risk of low participation volume from clients was identified, but there was no contingency plan for this in terms of additional marketing. Another team memberââ¬â¢s paper was clear in how customer needs could impact the success of the strategic plan. Internal and external factors addressed employee relations and shareholder expectations. The approach the company needs to take with delivering service to customers is identified as factor that needs to change as the plan is implemented. A SWOT analysis would be conducted and contingency plans made on an as-needed basis. The company may want to develop several contingency plans for common issues within its industry to be prepared for these scenarios. The third draft reviewed focused on short and long-term objectives and the use of a balanced scorecard approach to evaluating and monitoring the planââ¬â¢s implementation. The critical components for objectives, functional tactics,à action items, and resource allocation were identified. Major changes to the organizational structure and management positions were key factors to the companyââ¬â¢s plan. Conclusion Although all team members did not post drafts or offer feedback those that did provided details pertinent to the assignment. Team members identified the items outlined in the assignment and appear to have strong knowledge of what a strategic plan needs to address to be successful in meeting goals.

In my past clinical practice, only some simple assessments were conducted as it is able to reduce the affect of the problem but not solve it. After studied nursing health assessment, some more extensive and specific assessments should be done to identify patientââ¬â¢s health status. The actual problem, strengths deviations and the risk of the health problem are explored at a detail and in-depth way. This article is going to discuss about the specific nursing health assessment for a patient suffered from abdominal pain, who was met in my past practicum placement. Case scenario Ms. Ma, Age 54, housewife, admitted via AE and complained she was having abdominal pain for 5 days. Sharp pain starts at mid-abdomen and then at right lower quadrant. Level of pain increased when coughing. She had had Panadol 500mg an hour ago but pain can not relief. Nausea and vomited small amount of undigested food twice in the past few days. No diarrhea. She feels tired but can not sleep because of the sharp pain. Her vital signs are: pulse 98, blood pressure 148/85 mmHg, temperature 39.2oC. Her skin is warm and dry. Rebound pain occurred at the right lower quadrant of abdomen. She has hypertension and need to take medicine 2 times per day. Ms. Ma was diagnosed with acute appendicitis. Keep NPO and IV 500ml normal saline is established. Blood test, abdominal X-ray and ultrasound abdomen are planned. Assessment of abdomen In the past clinical practice, I only give analgesics by doctorââ¬â¢s order and the patient may sometimes relief pain after medication. However, abdominal assessment skills are necessary to identify Ms. Maââ¬Ës condition for getting at the root and having a better outcome. There are five important steps for evaluating abdomen: take health history, inspection, auscultation, percussion and palpation. These assessment skills will be discussed one by one in the following paragraphs. à Health history and lifestyle health practices First, find out the patientââ¬â¢s chief complaint, record the details and observe Ms. Maââ¬Ës general appearance. Then, assess the abdomen pain by COLDSPA- character, onset, location, duration, severity, pattern and associated factors. It is the most accurate measurement to identify whether it is parietal peritoneal pain, visceral pain or referred pain (Judy, 2008). After that, collect individual and family past and current health status. Ask if there was any injuries or trauma may cause the pain, any eating disorder, any abdominal surgery was done before, any food allergy, history of suffering inflammatory bowel disease, family history of cancer and chronic disease, etc. Also, collect Ms. Maââ¬Ës lifestyle and health practices. Ask her if smoke, drink or not, her eating habit, bowel pattern and movement, the amount, colour and texture of stool, any change in appetite, weight and abdominal girth recently and her stress level (Medical Education, 1998). Past history and current lifestyle health practices are the useful information to identify the risk factors of the problem. After collecting all background information, the physical examination should be proceed. Physical examination is using senses to collect objective data. It is used to identify the actual and potential health problems, discover patientââ¬â¢s abnormalities and diagnosis the problem (Nursing 2010 Magazine, 2010). For physical examination of abdomen, Ms. Ma needs to empty her bladder first in order to avoid the bladder irritation then, place Ms. Ma in a supine position. The hands should be at aside and knees slightly bent. Tell her keep relax of the abdominal muscles. The assessment should be started in the right lower quadrant of abdomen and then proceeding in a clockwise direction. Also, the examination should go forward in the order of inspection, auscultation, palpation and finally percussion for avoid affecting the quality of bowel sound and increase peristalsis (Bellack, 1992). Physical examination of abdomen Inspection Inspection is systemic visual examination. For abdominal examination, it should be started at the mouth, which is the beginning of gastrointestinal tract, and finally the rectum and anus (Bellack, 1992). First, ask Ms. Ma opens her mouth and says ââ¬Å"Arâ⬠or use tongue depressor to inspect the structure of mouth cavity to see whether any inflection, ulcer or not. Then, give a swelling test to Ms. Ma for examine the swelling ability. Place a spoon with some water on the middle part of her tongue and ask her to swell the water slowly to observe any choking or water leaks out. After that, inspect the texture of abdomen, the condition and colour of skin, any bruises or scars presence on abdomen. Normally, abdomen is homogenous in colour. If redness or yellow orange appear, it may indicate inflammation or liver disease respectively. Normal abdomen should also be symmetry from side to toe, flat and have normal movement when smooth respiration. If the abdomen is asymmetric, obesity, abnormal enlargement of organs, fluid distention or even intestinal obstruction may be suffered. Also, aortic pulsation should be present as Ms. Ma is having hypertension. Finally, ask Ms. Ma to take a d eep breathe and hold it, it is used to inspect the presence of hernias or not (Bellack, 1992). Auscultation Auscultation of abdomen is used to define the bowel sound, which are caused by the movement of air or fluid at small intestine, by stethoscope. The examination is started at the right lower quadrant, where the clearest bowel sound can be heard. Normal bowel sounds are at high-pitched, bubbling sound and occur five to thirty times per minute. If hyperactive bowel sounds occur, it indicates diarrhea or early stage of gastroenteritis. If hypoactive or even absent of bowel sounds for five minutes, it indicates intestinal obstruction, peritonitis or pneumonia. Besides bowel sounds, vascular sounds of aortic, renal, iliac, and femoral arteries can also be auscultated. It is an important examination to assess hypertension patient such like Ms. Ma whether she suffers from portal hypertensive and liver cirrhosis or not. If the vessels constricted or dilated, a bruit can be heard when blood flows (National Institute for Health and Clinical Excellence, 2008). Palpation Palpation is using sense of touch to collect data. For abdominal examination, finding out the location of pain is a great help of diagnosis abdominal pain. Light palpation and deep palpation are used to assess the abdominal organs, to define the tenderness and presence of mass. It is essential to assess the liver and spleen in abdominal examination. Light palpation which is not more than 1 am deep on each quadrant. Normal abdomen should be smooth and consistent. If broad-like hardness appears, it states peritoneal irritation is suffered. Deep palpation, which is press deeply from5cmto8cm, is used to indicate the abdominal organs and detect some obscure masses. Palpate the liver to test Murphyââ¬â¢s sign of cholecystitis. Palpate on the right upper quadrant at midclavicular line and parallel to the midline. If Ms. Ma feels pain and has inspiratory arrest, it states positive Murphyââ¬â¢s sign and indicate cholecystitis. Then, palpate the spleen at costal margin on left upper quadrant to feel if the spleen is enlarged and Ms. Ma will feel pain when the peritoneum is inflamed. Finally, as the rebound tenderness was being tested to Ms. Ma, that is pushing 90o angle at the right lower quadrant deeply and then release quickly. It is the reliable test of peritoneal inflammation if the patient feels sharp pain when the force released (Watkins, 2010). Besides, obturator test and iliopsoas test can also be done for diagnosing appendicitis. For obturator test, Ms. MA need to hold her right leg above the knee at 90o angle, grasp the ankle and rotate her leg laterally and medically. If she feels pain, it states obturator muscle is irritated. For iliopsoas test, straight up Ms. Maââ¬â¢s right leg and press deeply on her upper thigh and ask her to oppose the pressing force. If she feels pain, it states that she is suffering from appendicitis (Beltran, 2009). Percussion Percussion collects data by vibrations and sounds. For abdominal examination, percussion is used to assess the amount of fluid or gas, the location of mass, the size of liver and spleen. Normally, tympanic sound is found at hollow organs such as stomach and intestine; dullness sound is found at liver, spleen or masses. To estimate the liver is enlarged or not, the normal distance of liver is 6 to12cm, which depends on the body size and gender, at the midclaricular line. To estimate the spleen by percussing behind the left midaxillary line. If the distance is greater than7cm, it states that the spleen is enlarged due to infection, mononucleosis or trauma. Moreover, test of shifting dullness and fluid wave to assess ascites. If the ascites of abdomen is more than 500ml, shifting dullness will be found. Normally, tympany is produced at abdominal midline (Bellack, 1992). However, for the abnormal case, dull sound is produced because of the cumulated fluid. Ask Ms. Ma rolls to right side and percuss from top to bottom. If the fluid is present, sound will change from tympanis to dullness and fluid wave will be generate when percuss on a side of the abdomen. It also has great variate in the abdominal girth. Documentation After the physical examination, documentation is necessary for the findings and development of care plan. Current of illness Ms. Ma states that her abdominal pain started five days ago. On the pain scale from 0 to 10, as 10 being the worst, she rates her pain is 7. Sharp pain occurs at mid-abdomen and then at right lower quadrant continuously. Level of pain increases when coughing. She has no known drug allergy and food allergy. She had Panadol 500mg an hour ago but pain can not relief. Nausea and vomited small amount of undigested food twice in the past few days. She has loss of appetite and lost about 3 pounds of body weight. No change in her abdominal girth. She has no diarrhea. She feels tired but can not sleep because of the sharp pain. She is having fever as her vital signs are: pulse 98, blood pressure 148/85 mmHg, temperature 39.2oC. Past health history and lifestyle practice Ms. Ma is a non-smoker and non-drinker. She has hypertension and need to take medicine 2 times per day. No abdominal surgery was dome before. She denies any injury or trauma occurs recently on her abdomen. She does not have history of suffering inflammatory bowel disease or family history of cancer and chronic disease. She states that her eating habit is health and the amount, colour, texture of stool are normal, but constipation sometimes. She does not feel stress or depression. Physical examination Ms. Ma has normal structure of mouth cavity and good swelling ability. There is no bruise or scar presence on abdomen. Her abdomen is symmetric and homogenous in colour. Her skin is warm but dry. By using the stethoscope, her bowel sounds are normal and no bruits are heard. Ms. Ma has rebound tenderness at the right lower quadrant of abdomen, pain occurs at obturator test and iliopsoas test when palpation.à Normal tympanic sound is produced at abdominal midline when percussion. Black culture is no stranger to stress and suffering, and in fact has developed sophisticated techniques to manage intrapsychic survival and to avoid survival fatigue. Bell argues that inner-city Blacks, in particular, deal with chronic trauma (family separation, rejection, illness and death of parents, severe illness and injuries throughout life, school and residence shifting, unemployment, divorce, pregnancy complications, war and disaster). Goals are frustrated and integrity threatened by loss (Bell, 1982, p. 1018). Surviving this kind of life is exhausting and the continuous onslaught of stressful events challenges a personââ¬â¢s ability to focus on the problem, analyze the situation, plan creative solutions, and remember their competen cy at doing so and their hope that this too can be survived. In responding to a stressful event, an individual needs to change or escape from the stressful environment, develop new skills for action, protect against overwhelming emotions with defense mechanisms, and recover from the event and its outcomes. Altering oneââ¬â¢s states of consciousness furthers all four of these requirements (Bell, 1982, p. 1018), and is therefore a critical skill to develop. Traditional Black cultureââ¬â¢s approach to healing involves altering consciousness to handle natural and supernatural persecution, central to cultural cosmology. It also involves divination and possession techniques to insure natural harmony with the universe. Black Christian culture uses prayer and testimony and being filled by the Holy Spirit in a similar way, expanding consciousness. Blacks use dancing and music and singing to alter consciousness, as well as physical exertion, masks, drugs, and fasting. Altering conscious ness is a tool for survival, establishing harmony, protection, creative solutions, and change. The article on psychotherapy is entitled, Dyadically Expanded States of Consciousness and the Process of Therapeutic Change. Tronick (1998) discusses the importance of emotional connection and inter-subjectivity, from infancy, and application to the therapeutic relationship. Without emotional connectedness, the mental health of the infant is severely damaged. Tonick (1998) introduces an hypothesis, to explain this, the Dyadic Expansion of Consciousness, Hypothesis, based on his Mutual Regulation Model (MRM) of infant-adult regulation. The MRM is a micro-regulatory social-emotional process of communication that generates dyadic states of consciousness. This infant-adult process is applicable to the client-therapist relationship, as well, and if the dyadic states of consciousness is generated successfully, therapeutic change will likely be forthcoming. Interpreting therapeutic material is no t enough. The infant must regulate to maintain balance, physically and emotionally. The adult is part of the infantââ¬â¢s regulatory system, as critical a part as any internal part. Maintaining homestatic balance is a dyadic collaborative process (Tronick, 1998, p. 293). For example, to regulate body temperature, the infant can kick off a blanket, change positions, become more or less active, or can use crying and other dyadic communication to be picked up and held comfortably by an adult.
